The EC&R Physician Assistant functions autonomously within agreed upon scope of practice to manage patient care needs for those patients with severely debilitating life-threatening illnesses or who are at the end of life. Primary care provider for the 42-bed Community Living Center (CLC) unit that treats a wide variety of patients. Responsibility to diagnose, treat, and provide care for this complex set of patients. Must diagnoses acute problems and provides effective treatment. Works with other providers by referral and consultation with patients who require acute critical interventions and are in need of transfer to those specialized areas. Works in an interdisciplinary manner with the CLC Care team to assure adherence to administrative and clinical functions of the program. Conducts peer reviews, implements service agreements, analyzes performance measures, serves on committees and assists the CLC team in maintaining statistics and data required to meet CLC and quality performance measures established by VISN/VACO. Completes history and physicals exams, discharge summaries, regulatory visits and PRN visits with documentation. Works closely with the Medical Director and other Advanced Practice Providers (APP) and the service line director. Provides in-depth patient and family education to allow for informed decision-making, assists in identifying and managing patient/family conflicts related to goals of care plan. Conducts patient and family conferences to determine goals for patients. Identifies symptoms causing physical and/or emotional distress and establishes plan of care to address needs. Utilizes advanced practice knowledge to manage complex health care needs of patients with serious, advanced illnesses in the inpatient settings. Serves as consultant to attending medical teams in the acute care area for symptom management, establishing goals of care and communication on end of life issues. Provides education in EC&R to Geriatric Fellows, PA/NP students, and Internal Medicine Residents. Work Schedule:

Education:
Graduate of a PA training program, or of a surgical assistant program which is accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor organizations; AND A Bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the U.S. Department of Education. Certification:
Current and continuous certification by the National Commission on Certification of Physician Assistants (NCCPA) is required for employment in VHA. English Language Proficiency:
PA's appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d). Grade Determinations:
In addition to the Basic Requirements, the following criteria must be met in determining the appropriate grade for appointment:
Full Grade:
None beyond the basic requirements. Intermediate Grade:
One (1) year of professional practice as a PA; ORSuccessful completion of a PA post-graduate residency program accredited by ARC-PA or by the U.S. Armed Forces; OR A Master's degree from a PA training program certified by ARC-PA or in another health related field from an accredited college or university recognized by the U.S. Department of Education. Senior Grade:
Three (3) years of professional practice as a PA; OR Two (2) years of professional practice as a PA and successful completion of a PA post-graduate residency program accredited by ARC-PA or the U.S. Armed Forces; OR Two (2) years of professional practice as a PA and a Master's degree from a PA training program certified by ARC-PA or in another health-related field from an accredited college or university recognized by the U.S. Department of Education; OR a Doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education. Chief Grade:
Generally, an individual in this grade is able to demonstrate the behaviors after 5 years of professional practice as a PA; AND have a Master's Degree. References:

The work is generally sedentary in nature with some daily intermittent walking, standing, bending, stooping, reaching, and carrying of lightweight items required. Rigid deadlines and need for high degree of accuracy periodically becomes stressful.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
